Subject: Notice of Data Security Incident and Steps You Can Take to Protect Your Information
I am writing to inform you of a data security incident involving a computer used in connection with my business, Integrated Insurance Agency, Inc. I take the protection of your personal and insurance information very seriously.
What Happened
On September 3, 2026, potential unauthorized remote access to my computer was identified. Upon discovering this activity, I hired a forensic analysis firm to do a forensic analysis on the computer to investigate the incident.
The key findings of the investigation could not find evidence of how the unauthorized access occurred, unable to determine whether any files were compromised, or whether information stored on the computer was viewed, acquired, or copied by an unauthorized party.
Because personal and insurance-related information may have been stored on the affected computer, I am treating this as a data security incident and notifying clients whose information may have been involved. I want to be transparent about what happened and provide you with information about the steps you can take to protect yourself. I sincerely regret that this incident has occurred.
What Information May Have Been Involved
The forensic investigation could not find evidence of information being compromised. As a result, the information potentially involved may have included some or all of the following:
- Name
- Mailing address, city, state, and ZIP code
- Telephone number
- Date of birth
- Medicare Beneficiary Identifier (MBI)
- Insurance member identification number
- Insurance plan name
- Insurance plan type
What I Am Doing to Protect Your Information
Upon discovering the incident, I took immediate steps to secure the affected computer and investigate the unauthorized access. I am continuing to take appropriate measures to protect client information and reduce the risk of a similar incident in the future, including:
- The affected computer was professionally examined and serviced for a complete and professional forensic analysis.
- Reviewing and strengthening password and access credential security.
- Evaluating existing security controls and implementing additional safeguards where appropriate.
- Working with compliance professionals to determine and complete applicable notification and reporting obligations.
- Notifying appropriate regulatory agencies and other authorities, as required by applicable law.
- Reviewing business procedures and information security practices to help prevent similar incidents.
I am committed to taking this matter seriously and addressing any identified security weaknesses.
